YPC Grant Final Report

Organizations that have received grants from the YPC and wish to apply again must first have submitted a final report for any previously funded projects or programs to be eligible for funds again. The final report deadlines may vary. Grantees should refer to their Grant Agreement to determine Final Report dates.

YPC Grant Final Report

  • Congratulations on receiving a grant from the Youth Philanthropy Council. Future funding is contingent upon receiving the outcomes from your project or program. Complete and submit this form, or return one you download from our website no later than two weeks after project completion or within one calendar year from the time of the award.   • Please include a copy of your final budget with this report. In general terms, please list the amount and sources - i.e. public appeal, grants, operational, fundraising, etc.

    Using this final report and the materials you’ve provided us (including photos), the Community Foundation of Southern Indiana would like to publicize on our website, social media, or other marketing materials the great work you are doing and how the Foundation is helping you do that. We will not include or publicize any financial information. If you would like to be a part of this please identify your preference below. Include your logo and any information you feel would be helpful when submitting your final report. If you do not initial we will not use this information in any public way.
